Application Preparation & Submission

We prepare and submit the Medicaid application on your behalf and serve as your point of contact with Medicaid agencies. We respond to requests, follow up proactively, and advocate throughout the review process.

Pathway

We help determine the correct Medicaid pathway based on your specific situation. This includes Community Medicaid for long-term home care, Immediate Needs Medicaid for urgent care situations, and Institutional (Nursing Home) Medicaid. We create a clear, customized roadmap so you understand which type of Medicaid applies, what documentation is required, and how the process will unfold.

Advocacy, Approval & Ongoing Support

Once financial eligibility is approved, we continue to work with you through the clinical evaluation process and final determinations. We advocate on your behalf and guide you through next steps, which may include setting up a pooled income trust if required, assisting with the selection of a Managed Long Term Care (MLTC) plan, and helping you choose an appropriate home care agency. Our support continues beyond approval to ensure services are implemented smoothly and families feel fully supported.
